- If our office participates with your insurance company we will file your primary and secondary insurances. If we do not participate with your insurance company your will be responsible for your entire bill.
- You will receive a statement only if a balance is due.
- If your insurance requires a referral, it is your responsibility to make sure you have the referral at the time of service, otherwise you will have to reschedule. If you fail to obtain the necessary referral, you will be responsible for your bill, in full.
- All co-pays are to be paid at the time of service. If you are unable to pay at that time, you will have to reschedule your appointment.
- You will need to bring all insurance cards with you on the first visit. Payment at the time of service will be requested if these are unavailable.
